Blake family photographs

Scanned photographs - originals kept by the family.

Includes portraits of Gerald Blake and his siblings and cousins. There is an image of Gerald Blake, Harold and Hume Wrong reading the news of the outbreak of war 1914. Some images are formal portraits while others show family members informally including at summer residences and in World War I uniform.

Additional material

Includes clippings from Miss Chatelaine article, "New Course on Campus: Women's Studies. Parts 1 and 2", Spring and Summer, 1972, by Myrna Kostash, scanned course calendars and outlines, flyers and additional newspaper clippings. Material also includes portrait taken at the interview.

Data

This subseries includes raw data in electronic form as well as Ms. Heaton's background notes on coding, computing and analyzing raw data in the SAS program.

Questionnaire

In 1993, Ms. Heaton conducted a mail survey to medical school library directors to gather information on reference services. This series consists of records documenting the questionnaire such as correspondence, draft questionnaires, and raw data. The series has been divided into subseries.

This fonds consists of one accession documenting Dr. Fred Baker’s association with the University of Toronto mainly during his period as Director of the Sioux Lookout Program (SLP). The accession is arranged into seven series:
Series 1 summarizes his career in curriculum vitae, and his appointment to the University and the Sioux Lookout Program.
Series 2 to Series 4 contain Dr. Baker’s personal files relating to his role as director of SLP and includes some lectures and workshop files, as well as correspondence relating to Indigenous health care.
Series 5 documents primarily his committee activities with the Canadian Paediatric Society and the Council of Faculties of Medicine of Ontario.
Series 6 contains Dr. Baker’s copies of two reports relating to Indigenous health.
Series 7 contains computer files of both personal files and official administrative files.

Gwynneth Heaton fonds

Fonds documents research undertaken by Gwynneth Heaton in 1993 and 1994 to investigate the provision of reference services in medical school libraries. The investigation examined factors that may affect reference service such sa the proximity of other desks providing information; the type of staff providing the service; the variety of services provided; the provision of research assistance by appointment; the physical proximity of a hospital; and the use of problem based learning in the medical school curriculum. The project involved a mail survey and follow up visits to selected medical libraries in Canada and the United States. The results of this research were published in various academic library journals.

The accession consists of correspondence, questionnaires, raw survey data, research notes and manuscripts of published and unpublished articles resulting from Ms. Heaton's research project.

The fonds is divided into two series.

Series 1 contains course notes, correspondence, addresses, articles, manuscripts, notes, minutes, and photographs relating to the activities of Thom Greenfield as a professor of educational administration at the Ontario Institute for Studies in Education and as a gay activist, especially in relation to "Gay Fathers of Toronto", of which he was one of the founders.

Series 2 contains professional materials that relate to Greenfield's appointment at the Ontario Institute for Studies in Education (correspondence with students and faculty, letters of reference, memoranda, and manuscripts), editorial work related to Greenfield's position as Associate Editor of "Curriculum Inquiry," manuscripts and correspondence related to the organization "Gay Fathers of Toronto," manuscripts for "The educational programs and purposes of the Batchewana Band: a management audit,"and personal correspondence and manuscripts relating to finances, politics, and family.

The accession contains Faculty Council meeting minutes and agenda and various material coming from committees which fall under the Faculty Council. The accession also contains Post Graduate Medical Education, Medical Radiation Sciences, and Undergraduate Medical Education Board of Examiners minutes and agenda. The accession contains student appeals and reviews, various superceded policies, and Faculty Secretary materials.

Digital files consist of minutes of meetings of the Academy Directors Committee, 2001-2004.

Correspondence

This series consists of files and electronic documents of general professional correspondence covering a period from 1991-2002. It documents various academic and political issues in which Prof. Rayside was involved. Much of the correspondence in this fonds relates specifically to documentation in other series.

Legal case files

This series documents legal cases Dr. Mastromatteo was involved in, usually in the form of providing testimony as an expert witness. All of the cases in this series are related to workplace illnesses and injuries.

Record types include reports, medical records, correspondence, papers, transcripts, court documents and notes.

Consulting

Professor Fuss has served as a consultant to government and industry for many years, but only two projects are documented in this series, his work as a member of the Price Measurement Advisory Committee at Statistics Canada and a study he did for United Communications Ltd. on long distance telephone service in Canada.
