File consists of blank copies of each of the following forms used in the interview process: File Checklist, Deed of Gift, Biographical Information Sheet and Informed Consent Form. With the Informed Consent Form is one copy of a Mauthausen Survivors Documentation Project Letter of Consent presumably used as a resource to create the consent form for this project. Two copies of the Interviewer's Checklist are also in the package which contain instructions for before, during and after the interview.
